ACA Form 1095-C (Health Coverage Statement)
Employees may access their ACA Form 1095-C directly through their Paychex employee profile on your landing dashboard. HRDC no longer automatically mails Form 1095-C. If you are unable to access your form and would like a copy, you may request one by contacting Human Resources at hr@thehrdc.org 406-585-4862. Requested forms will be provided within 30 days of the request.
